Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)

Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) is an evidence-based approach designed to support children, adolescents, and caregivers impacted by trauma. It helps young people process difficult experiences, reduce trauma-related symptoms, and build skills for safety, emotional regulation, and resilience.

TF-CBT is structured, supportive, and paced carefully—always prioritizing emotional safety and trust.

How TF-CBT Works

TF-CBT combines trauma-informed care with practical cognitive-behavioral tools. Therapy focuses on helping clients:

  • Understand how trauma affects thoughts, emotions, and the body

  • Develop skills to manage anxiety, fear, anger, or overwhelming feelings

  • Gently process traumatic memories at a safe and appropriate pace

  • Reduce shame, self-blame, and negative beliefs

  • Strengthen coping skills and emotional regulation

Caregivers are often included in the process, when appropriate, to help reinforce skills, improve communication, and support healing outside of sessions.

Who TF-CBT Is For

TF-CBT is commonly used with children and adolescents who have experienced:

  • Abuse or neglect

  • Medical trauma

  • Community or school violence

  • Sudden loss or grief

  • Accidents or other overwhelming events

It can also support caregivers who want tools to better understand trauma responses and help their child feel safe and supported.

What to Expect in TF-CBT Sessions

TF-CBT sessions are structured but flexible. Therapy may include:

  • Psychoeducation about trauma and stress responses

  • Emotional-regulation and grounding skills

  • Cognitive tools to challenge unhelpful or self-blaming thoughts

  • Gradual, supported trauma processing

  • Caregiver sessions focused on support, boundaries, and connection

The pace of therapy is always guided by the client’s readiness. There is no pressure to share more than feels safe.

A Trauma-Informed, Affirming Approach

At Small Works Counseling, TF-CBT is offered through an affirming and identity-aware lens. We recognize that trauma does not happen in a vacuum and that culture, identity, family systems, and systemic stressors all matter. Therapy is collaborative, respectful, and centered on the child or teen’s sense of safety and autonomy.
